Creation of Downlink to read modbus registers

Hi.
Following an certain data value received, i need to set up a downlink immediately to read some Modbus registers.
Can someone help how to configure same on Chirpstack 4?

I would write an external application/service to read from an integration (MQTT, probably) and write the downlink as necessary.
